Reflux: Sometimes acid reflux can cause upper chest discomfort after waking up. Try over the counter zantac for 3-5 days. If not better the consult your primary care physician.

C/p on waking: May be GERD/ reflux /gastritis. Would elevate head of  bed by putting pillow under mattress, consider ranitidine or prevecid for a month and see if makes a difference . Triggers are alcohol caffeine, energy drinks, smoking spicy foods. rarely may be pericarditis/cardiac due to autoimmune process, sarcoid ,but woud lbe short of breath, coud lcheck ESR ,CRP , EKG(more likely GERD)   f/u/ w/ doc

Not your heart: I'm assuming you're worried this is your heart. At age 28 unless family members have been dropping dead of heart attacks in their 20's and 30's there is no risk for heart attack at your age. The lungs, lung lining, esophagus, and the chest wall (composed of 100s of muscles crisscrossing between the ribs) all live in your chest and can be the source of your pain. Heart pain doe'nt last just seconds

It is  possible: that the upper chest discomfort could be caused by the GERD and the smoking does not help either. The pain in  the armpit is harder to explain so, see your PCP who could run  some tests and a chest X-ray to rule out other possible causes of your symptoms.

Cardiology 1st: I suggest you see your primary care physician for referral to a cardiologist who will do testing such as an ECHO cardiogram and exercise stress testing that is not done in the ER. If the inhaler does not help, don't use it. The ER mainly rules out arrhythmias and heart attacks, but not all cardiac problems. Asthma causes shortness of breath and wheezing. Is there a family history of heart disease?

See your doctor: There are many potential causes of chest pain, ranging from musculoskeletal to respiratory to cardiac/heart, among others.  Without a complete history and thorough exam, it's impossible to determine the cause of your chest pain.  You should make an appointment with your primary physician to determine what testing or treatment is recommended based on a complete assessment.

Anything: Anything is possible. However if it was "embolism" (I.m assuming you are referring to pulmonary embolism) then likely you would have other symptoms such as shortness of breath. This sounds musculoskeletal type pain however no way to be sure without a proper medical evaluation.
