Needs testing: Lumps, bumps, blisters and other skin abnormalities on the genitals need to be evaluated. An examination will be needed to know the cause and treatment. What you are describing is concerning for a herpes outbreak. Please get tested and avoid intercourse until you see a doctor.

Needs further eval: Was this ulcer cultured for std, was this ulcer a result of trauma(scratching) is there an underlying skin condition called lichen planus or/and lichen sclerosus. These conditions are treatable and can reverse the changes that you refer to. But needs to be well evaluated to begin with.

Genital ulcer: Herpes is the main cause of genital ulcer, but unlikely with both neg swab tests and no response to "antiviral" drug, assumin oral anti-HSV drug, e.g. acyclovir or valacyclovir. Second most common cause of genital ulcer is syphilis, which might temporarily respond to Keflex. Syphilis ulcers usually not painful but can be. Chancroid, another STD, is very rare. See infectious disease or STI expert.
