Vocal cords: Usually in women this is due to inflammation on the vocal cords.  Although often this is due to something as simple as post nasal drip or even reflux, it is probably worth seeing an ENT doctor for a look down the throat at the vocal cords!

Dysphonia: Poor technique. Tension in the voice box, poor breathing, stress can all contribute to make a significant change. See an ENT doctor and you may need speech therapy.

Ugly how?: There are many voice problems which are correctable.   If your voice is hoarse, raspy, or otherwise abnormal, then you should see your local laryngologist or ent.  If you smoke, stop immediately.  That will invariably damage the voice.  Other problems like laryngeal reflux and vocal abuse may be causing the problem, and can be corrected.  In some select cases, surgery may improve your voice.

Therapy?: Have you yet tried any form of psychotherapy? Typically, the optimal combination of things to treat emotional issues such as depression is medication (which, on your best day, is supposed to take the edge off symptoms) but the real gruntwork takes place in psychotherapist's consulting room.

Later in puberty: Voice change in males is dependent on puberty. As a male goes through puberty, the larynx gets bigger and the vocal cords lengthen and thicken, so his voice gets deeper. The completed change may not be noted until the end of puberty which approximately 3-5 years after it starts. For boys who started puberty later, this may carry over into their early 20's.

ENT doctor: You should see and ENT doctor and have h/her look at your vocal cords and advise you.  If you smoke then it is a "no brainier" as to what you need to do.  Still see the ent, which stands for ear, nose, and throat.

Deep voice: Unless you are taking testosterone your deep voice is caused by something else.

On the other hand, a pleasant deep voice in a woman can and will be appreciated by lots of folks.

If after both of these you are still concerned you might want to see an ear, nose, and throat doctor.

Fear?: It is possible sometimes to become extremely fearful, especially if this person has harmed you in the past and you feel threatened by him or her.   I have known people who could not speak when afraid -- it's like a "freeze" response.  If you've experienced a trauma, please tell your doctor and/or therapist about it.  Also keep yourself safe.

Get Help: Did your delusional disorder impact the relationship?  See you are on mood stabilizer, wellbutrin (bupropion) &geodon. What are you doing non-pharmacologically for the anxiety? Talking therapy ranks really high. Cognitive behavioral therapy or psychotherapy can be quite effective. Aim for daily exercise, 8 hours of sleep per night &good nutrition with 5 servings of veggies &4 of fruit per day. Try tai chi, .
