Double therapy?: Your brief history sound a bit strange unless something is missed there. With tubal ligation done there is no need in additional contraceptive therapy with Mirena (levonorgestrel) device. As far as symptoms you are complaining of, yes Levonorgestrel (active ingredient in Mirena (levonorgestrel)) may cause it. Go to our PMD or GYN, get qualified advice after your entire history is reviewed and you are examined.

Possible butunlikely: The Falope rings are highly effective at occluding your tubes and preventing pregnancy 99% of the time.  Unfortunately, an intrauterine or extrauterine pregnancy after a "tubal" can still rarely occur, usually for reasons no one can explain. Be safe. Ask your Gyn for a blood betaHCG pregnancy test which will probably be negative. Your symptoms may be due to ovulation or ovarian hormone problems

Tubal ligation ?'s: Fallop ring sterilization done "6 years ago" effectively destroys the segment of the tube where the ring is placed, by cutting off its blood supply.  Although this type of tubal ligation can be reversed, it is very unlikely to permit pregnancy even if it does "fall off".

Normal menses: Congratulations, you are experiencing normal periods which unfortunately can be heavy and crampy with premenstrual bloating, PMS, and breast tenderness.  All normal!  A tubal simply blocks the sperm and egg from getting together.  Your body thinks you want to conceive so prepares. If these symptoms are that bad, consider going back on a hormonal birth control which will eliminate these symptoms.

: Several things come to mind. One seems more likely, psychosomatic, your symptoms are based on emotions alone, there is no physical reason for the problem.  You may have a neurological problem with the finger. See a doctor.

If you start to have nause or shoulder pain, that could be a sign of severe internal bleeding and you should go to the er immediately.
