See below: For many people that can improve with time. If it has been more than a month and your heart rate remains elevated (above 100) i would actually consult a cardiologist. They should at least get an ekg to see the electrical conduction of the heart. There is a group of medications called beta blockers that are great at reducing heart rate. But this should be evaluated further first.

Heart: It is possible that the left side or main pumping chamber of your heart is not getting enough blood and the most likely reason for that is a blockage in one or more of the arteries on your heart.  There are several ways to test for that such as and ekg or a stress test or a ct scan or MRI of your heart.  Treatments vary depending on the severity and number of the blockages.

Low EF: an EF of 26% requires an etiologic diagnosis first and then treatment of the cause. If its a cardiomyopathy without surgical remediation, the treatment would usually include ACE inhibitor, beta blocker, perhaps spironolactone type medication, perhaps diuretic and other things also possible. The diagnosis and patient status determine the choices.

It depends: I am assuming you mean congestive heart failure.  This can be caused by several things.  Treatment will depend on what is the cause of the heart failure.  For ex.  Valve disease, blocked arteries, high blood pressure, infections can all be a source.  If the problem is treated it will often help.  Certain medications like diuretics, blood pressure meds, limiting salt in diet are often used too.

In some cases, fluid (if present) may be withdrawn from the pericardial sac for both diagnosis and relief of symptoms. It may be necessary to open or remove the pericardium in more severe cases.

In the acute setting, antibiotics to treat the strep infection, Aspirin to decrease inflammation, and occasionally steroids are used.  In most cases, the valvular scar caused by rheumatic fever are minor, and does not require intervention.  However if the regurgitant (leaky) or stenotic (narrow) condition is severe, surgery may be indicated.
