No standard cost...: In the US there are no standard prices for procedures. Ketamine injections are not a routine treatment here so it may be difficult to even find a doctor who offers this. If you do, you would need to call the doctor's office to ask but any doctor would need to first do an initial patient visit and it would undoubtedly cost much more than $50. Health care in the US tends to be rather pricey.

Immediate Evaluation: Severe depression can lead to self destructive behavior (suicidal) Immediate psychiatric evaluation is obviously important, Sometimes hospitalization is first step even if it is short term. There are excellent antidepressant  medications but meds often take a few weeks to work sometimes combined with other meds. Psychotherapy is frequently essential which can deal with short and long term issues.

Some cities have such clinics.  It would not be a maintenance medication that someone could use at home.

The treatments: you describe are dramatic and not commonly available.
I have had migraines and know the severity of pain. I suggest that you see a neurologist and be seen on a consistent basis for treatment. Are you taking too much caffeine? Have you treated your stress? More recently have noted reports that the botox does little for the pain. The neurologist should be up on the latest treatment.
