Arthroscopy: Since you are "very fit" you should be back to full fitness in around a month after surgery. The standard time is 6-8 weeks after surgery.

Probably: There are many causes of hip pain= osteoarthritis, labral/cartilage tears, hip flexor strains, bursitis, stress fractures & referred pain to the hip area, often from the lumbosacral spine. Stationary bike positioning often puts the hip into a significant amount of flexion, irritating the joint & soft tissues. Consider elevating the seat of the bike to reduce this flexion. See your doc for an eval.

Total hip replacemen: Once the bones have necrosed there are really only two option:  1- treat the pain for rest of your life. 2- total hip replacement should be considered.

See your doctor: It may be best to first start with your pcp, and discuss the concerns you have. So that you have a starting point. Then s/he can also do a hormone panel assessment to see how you're doing to ck your Progesterone and estrogen levels. The severe hip pain needs to be assessed further too. So that may be related, in that hormones have a protective effect on bone, heart, and brain. But it may also be that it is completely separate, and unrelated to your hormones. So this is why you need to speak to your doctor.

Generally speaking weight loss would be of benefit but oftentimes weight bearing may make it worse non weight bearing exercises such as swimming or stationary bike could be beneficial.I would see your pcp, : sports medicine doc or orthopedic physician.

AVN of the Hip: If you have true avn of the hip then you need to see an orthopedic surgeon. There are various stages of avn. In the early stages there is a remote chance of salvage of the hip.  If the avascularity is too far advanced then you need to discuss treatment options with your orthopedist. As you are only 40 years old, if you need a hip arthroplasty it is going to have to last quite some time.
