Bruise: For the first 24 hours, i would ice the effected area to minimize the spread of the bruise. After 48 hours, heat may be applied to allow the blood to disperse about the body. The amount of time it takes to heal is highly variable from person to person.

Back pain: Probably a muscle strain or pull however it is possible you may have a fracture.  If it does not get better quickly you may want to get an xray

Hematoma: A deep bruise can result in bleeding down in the tissues call a hematoma.   A lump and tenderness can persist for weeks-months after the "bruise" goes away.  But it usually slowly gets better.  If there are persistent concerns, see your doctor.

Stop anticoagulants: Bruising is caused by bleeding and implies coagulation problems. Aspirin, nsaids, coumadin, (warfarin) plavix, xarelto, some herbals, and many others lead to bleeding. Check for these causes. Otherwise need a hematologist to check blood for lack of clotting factors as easy bruising/bleeding is not a good or natural sign, especially in a young person. Limit trauma.

It could...: ...But it usually is not.  Most bruises are caused by injury, or by the sun thinning the skin around veins, making them fragile. But bruising can also signal a bleeding disorder or an underlying medical problem; so if your bruises are appearing out of nowhere for no apparent reason, you should see your doctor.

Healing: If it is a very bad bruise, cells are damaged and release chemicals that may cause itching, such as histamine from mast cells. Also as a wound, the bruise, heals it contracts and this causes itching from the contraction.

Treating bruise: Initially you can use cold compress to keep swelling and bleeding down. When swelling is gone and bruise stops changing, use warm compress to help remove the color.  I don't recommend to use heat unless it is on your face and you want it to go away quickly. Heat can make any swelling worse.
