Abdominal ultrasound: Transabdominal ultrasounds ordered by your physician could be done at most hospital radiology departments or at large medical groups. They require that you have a full bladder. 
Normally the ordering physician would make recommendations at to choices of facilities to perform the ultrasound.

None: The examiner should have an order for the test being performed and should not need anything specific from you other than any pertinent history relating to the reason for the test.  If you have had organs removed (ovary, uterus, kidney, gall bladder, etc) the sonographer will want to know.

Not likely: Ultrasound dating is quite accurate in the earlier part of pregnancy.  While it may be off by 2-3 days it would be highly unlikely to be "wrong" by 2 weeks.  Keep in mind that the ultrasound age indicates the number of weeks since the first day of the last menstrual period, not the date of conception. This may explain a two week difference if you arethinking about date of conception.

See doctor: Did the ultrasound confirmed the diagnosis of placenta previa? If so, the bleeding is most likely due to the previa itself since the ultrasound was a transabdominal one and not transvaginal. If you have placenta previa and are having vaginal bleeding, please see or call your doctor immediately, as it may progress to preterm labor, and you will need a c-section and other emergent medical care.

Depending on the: Quality of the machine and on your anatomy, generally about 6 1/2 weeks.  It's about 5 weeks and 5-6 days on transvaginal.

Access point.: Both transabdominal and transvaginal sonogram are useful and may even be complementary in imaging the pregnant uterus or non-pregnant pelvis. Different probes are used for each type and the transvaginal approach is clearly less comfortable, however it has the advantage of improved imaging in obese women, in early pregnancy and in fetal brain lesions in late pregnancy.

No: The fetal heart begins to beat at or after 5 weeks. Whether or not you would see it would depend on the skill of the technician doing such a study.
