Several things: Hi. Either you&#x27;re having intravascular hemolysis and hemoglobin&#x27;s coming out in the urine, or there are red cells in the urine that measure as urinary hemoglobin. I&#x27;d recommend a urine analysis with microscopic evaluation to see if it&#x27;s reproducible. If no RBCs in urine, Check serum free hemoglobin and haptoglobin, and have a pathologist look at a peripheral blood smear. Good luck!

It's random: Each time you conceive a child, a random assortment of genes from each parent goes into making up junior's genome. This is basic biology but still often needs to be reviewed. It's good for folks with any thal variant to take enough folate, (folic acid) but you should be taking that iron if and only if you are actually iron deficient or clearly need to supplement. Looks normal: These levels do not seem particularly abnormal for a pregnant woman. Contact the ordering physician and ask what was abnormal and what your instructions are. They may want to start you on an Iron supplement for mild anemia.

Chronic problem: Is the doctor a specialist- hematologist? He/she is hopefully explaining  this as you go. Your child will have chronic anemia and may need transfusions to keep the hemaglobin up. That can cause iron overload so that has to be handled as well. The ebeta thalassemia can be mild or severe and it is hard to know the severity of this in your child.

at 28 weeks pregnant my wife was found to have low ferritin (9 g/l) but normal hgb (123 g/l). her iron wasn't tested before but hgb was 135 g/l at 11 weeks. can we infer if iron was low before and if there was a risk to baby's brain development?

: Not necessarily. We find that the placenta and babies other systems selectively take from mothers body during pregnancy to get what they need while leaving mom depleted. There can be a point, marked by poor baby growth parameters for gestational age where it could be. However, moms body is doing much of the metabolic work for the baby so it rests &amp; grows. This is not that worrisome.

my husband has pv that is controlled through phlebotomy &baby aspirin. his hgb was down to 11.3 and hct 38%. his dr put him on iron. after 1 mo. of iron his hgb is 14.6. doctor wants him to continue for 2 more months. won't this raise him too high?

: It often takes several months of iron supplementation to build the bodies iron reserves to an adequate level after anemia. The bodies own system of iron absorption begins to lessen when good stores are present. This treatment plan is standard of care and will not produce the "TOO HIGH" issue you propose.
