See a GI doctor: Blood in the stool is never normal and can signify severe illness, although it may be due to hemorrhoids, anal fissure or other minor conditions. The presence of episodes of fever, however, reinforces the impression that it should definitely be evaluated by a physician and would recommend this be done asap, particularly in light of prior history of tuberculosis. Need specialists: The symptoms are not specific to a disease but may be from a number of benign or self limiting inflammatory or infections origin so a visit to a gastroenterologists with complete history exam and perhaps tissue evaluation may clarify your disease if it is one of chronic non specific types or more specific type.

Mild; get checked: If uc is suspected, need a colonoscopy/biopsies to check.  More limited exams like flexible sigmoidoscopy will tell if rectum and sigmoid are inflamed; anoscopy will tell if distal (far end) of rectum is inflamed.  Uc starts in rectum, heads north.   Mild symptoms, mild cases don't have fever, pain, blood.  If uc is brewing, and undiagnosed, untreated, can develop the above symptoms and more.

Urgent care-hospital: The history suggests active bleeding in the GI tract.Urgent assessment is recommended.The fever is a complicating issue.The black stool suggests old blood or blood that has been affected by gastric acid ,it is still blood.Please seek help and mention a travel history if relevant.

Multiple: Many times if the child is constipated there will be a bit of blood from an anal fissure ( cut) . Sometimes it may mean some viral infection. Sometimes it may mean a food intolerance/allergy. Please see your pediatrician, and bring a stool sample.
