At least 3 weeks: The fastest i would taper 450 mg of lithium would be 150 mg decrease per week for 3 weeks. Gentle tapers are the safest with lithium, since discontinuation can provoke relapse of mania or depression. If coming off lithium is clinically indicated, it should be done under supervison of your physician and the fastest taper would be 3 weeks. In my own practice i would stretch to 6 weeks.

Depends: Assuming your doctor know hoe long you have been on it &why you are taking it, this is reasonable. You will have some "withdrawal" symptoms like maybe not sleeping as well or maybe a return of some of the symptoms of what you were taking it for.  50mg is a relatively low dose so this seems to be reasonable. Also, if you are being changed to a new med, you may be starting it while reducing it.

i'm not sure how the word "titration" is used. people can titrate a dose upward, but if a dose is gradually decreased is that "titrating" downward?

Just stop taking it.: 12.5 mg is the lowest dose of Toprol (metoprolol) XL available. Assuming u take it once per day, the tapering schedule would be to just stop. If u r taking it twice per day, then switch to once per day for 2 or 3 days, then stop (u cut total 24-hr intake by 1/2 for 2-3 days, then in 1/2 again for 2-3 days, etc. until u are at the lowest dose; then stop).

Get in.: the imipramine would be started at allow dose like 50mg every night and then slowly increased over time.  If you are considering to restart your medicine, make sure you discuss how and other possible options with your physician.

Yes: It is generally advisable to reduce doses gradually over 1 - 2 weeks before discontinuing to reduce/avoid rebound hypertension/ heart problems. Please work with your doctor for a safe discontinuation of toprol (metoprolol) particularly if you have been on it for a long time for hypertension treatment. Report any irregular heart beats, increase anxiety/ dizziness/fainting when discontinuing toprol (metoprolol). Take care.

my husband has been taking cymbalta (duloxetine) for years, but thinks it might be doing more harm than good and would like to titrate off. is that safe?

Consult doc..: It is best to consult/inform his doc why he wants to stop med and ask for weaning instructions etc. This med should be weaned off over couple of weeks, otherwise he will likely experience some unpleasant symptoms of withdrawal if discontinued abruptly..

i can no longer afford cymbalta (duloxetine) for pain. it's not advised to stop abruptly. can i open the capsule & remove some of the beads to titrate down?

Yes: I tell my patients to do exactly that. You then close capsule and take the cymbalta (duloxetine). Make sure you wash your hands well afterwards and dry them well so as to avoid transferring any of the medication accidentally to your eyes. Make sure your prescribing physician is consulted first .Best wishes.
