i recently notice there were these small white pimple like on the inner part of my foreskin. it's not big just really small spot. what could it be?

Penile pimples: Are they white or flesh colored ? are they on the corona? If flesh colored on the corona could be pearly penile papules which occurs in uncircumcised males and is nothing to worry about. Could also be Fordyce spots -benign sebaceous cysts  or actual pimples don't  pop as they can get infected-again  not to worry  .F/u w/ doc sometimes topical  antibiotics make it resolve, otherwise not issue.

: It is not feasible to provide a meaningful opinion without additional history, physical examination and may be some tests. What you described suggests a genital wart. It would be prudent to see a doctor. Wish you good health!

Possibly: White dots on the head of the penis or on the foreskin are usually oil glands which sometimes can become larger and more prominent then go down again. If they weren't there before then i'd have them examined by a medical professional.

what can the patch be and how should i treat it?

Foreskin: This could be an early manifestation of a condition called lichen sclerosus which is usually itchy. If this is what it is, it can progress and eventually result in phimosis from scarring unless it's treated. I recommend consulting a dermatologist or urologist for proper diagnosis and treatment.

No question posted: ... but Fordyce spots are consider a normal feature for many people. Therefore, one may have to consult with several urologists in order to find one who might be interested in treating them. A skin laser treatment center may be needed, so a dermatologist consultation may be helpful. One should realize that removing a spot can leave a scar. That said, first get a diagnosis from a doctor.

Sounds like Herpes: You should be seen by a primary care provider for evaluation.  A skin biopsy may be necessary, blood work can help too. Typically herpes is treated with oral antiviral pills- acyclovir, valacyclovir, etc.  There is no cure, but it is treatable and controllable.  A drug eruption is also possible at that location.

: These are descriptive terms. In an uncircumcised male the outer foreskin is the outside skin that covers the end of the penis when the glans is not exposed.  The inside foreskin is in direct contact with the glans. They are the same layer, just described in the context of their location.

Balanitis?: It sounds like you have balanitis, the medical term for inflammation of the head of the penis and the foreskin, especially if in addition to "cuts" there is redness, whitish patches, or odor. It might be "nonspecific"; or possibly a yeast or bacterial infection. See a doctor. Once healed, prevention is usually just normal hygiene, i.e. retract foreskin and wash once or twice daily.

If uncomfortable: Your foreskin is only too small if it is tight when retracted to the extent that it is painful or uncomfortable. A tight foreskin is called phimosis and in adults is best managed surgically.  A phimotic foreskin can certainly tear if retracted with force. Steroid cream can be tried in prepubertal boys. A short foreskin by itself is no problem provided it's loose.

Be checked and see..: Most likely, you have long tight foreskin - phimosis. If unbearable despite local care including topical use of anti-fungal/steroid cream under professional instruction and watch. you would need circumciosn, which could be done under local anesthesia as an outpatient. So, see and discuss with a urologist, and decide what you really need.  Best wish...
