Bronchospasm?: Please see an acute care center or your PCP right away. Many conditions may result in these findings. An EKG 12 lead and considering bronchospasm as a cause is recommended.

Other symptoms?: If you are having only intermittent chest discomfort, not related to exertion, it may be musculoskeletal.  Changes with your meal pattern may point to a gastrointestinal cause.  If you have risk factors like diabetes, smoking, high cholesterol or hypertension, your overall risk is increased, however it is best to have your doctor examine you to determine if your need any further testing.

Might be an ulcer: your symptoms suggest that you might having an ulcer or gastritis it could a side effect to the surgery that you had some of the side effect of the reflux surgery of course are bloating delayed gastric emptying that could lead to ulcer or gastritis i am assuming you had your motility study and esophageal pH and gastric emptying study before your surgery
you need to see your GI doctor

Esophagitis : You did mention in your history that you have acid reflux. There is a muscular valve in between the bottom of your esophagus and your stomach. If the valve is mechanically weak then stomach acid will leak into the esophagus when lying down because the effect of gravity is partially cancelled when flat (vs vertical:standing upright). Try putting 1-2" block under each headboard side of bed; may help

Very unlikely: Risk for cardiac problem very unlikely unless you are using substances such as cocaine. Given your recent upset stomach I would be suspicious for reflux as a more likely cause for your symptoms. .

Multifactorial: Chest tightness and pain can be caused by a variety of reasons, including cardiac, pulmonary, gasto-intestinal and musculo-skeletal.  In order to differentiate the cause a careful history and physical exam is often necessary. Lab tests and radiologic studies may also be necessary.  The answer is often easy but can be very complicated.

Chest oressure: The most serious issue is heart disease. If that is ruled out it could be anxiety, structural problems, gastrointestinal problems.

Chest tightness: At your young age chest tightness is most likely due to anxiety and tension but more information is needed to exclude rib cage inflammation or strain, pleurisy,esophageal spasm,etc. Your physician can evaluate you and advise you best.
