It means that the-: -articular cartilage in the joint is worn down to bone on bone in some areas and almost there in others grade 4 =bone on bone, grade3= almost there.

Tough to answer: The majority of tibial plateau fractures do not involve the lateral ligamentous structures. In general, the lateral structures (fibular collateral, popliteofibular, popliteus, etc) require surgery to repair or reconstruct. I think your case is much better addressed with a specialist and careful review of the situation.

No : Hernia is from a defect in the muscle covering. It can be repaired in certain situations. See a specialist.

Muscle hernia: Treatment for tibialis anterior muscle herniation is controversial. Asymptomatic hernias require reassurance, but no specific treatment. Minimally symptomatic hernias may respond to conservative treatment with restriction of exercise and the use of an elastic support].  Traditional surgical options involve direct closure of the defect.

Possibly yes, probab: ly not. 
All injuries, surgeries and the Infection would have left a lot os scar tissue which can invove the nearby nerves, causing tingles or pain etc,
Or over the last 18 months or so you could have developed some back issues or which became obvious recently that you are having problems now.
Certainly talk it over with your surgeon and He'll either help you or direct you to the Right person.

Timing is everything: The simple answer is yes. If not operated on, pt with rom and quad strengthening will be initiated once your doctor feels it is stable. Following surgery, pr is frequently started earlier as long as the fracture is stable. Healing time usually is 3 to 4 months.

Go slow: Lots of possible causes of tibial plateau including painful hardware, stress fx, inflammation, meniscus tear, arthritis.....I would suggest going slow.
