Check it out.: You didn't mention if the nipple inversion is a new finding. White discharge from the nipple usually turns out to be non-problematic, but I am concerned because you have pain and the inverted nipple. Prolactin: This can be galactorrhea. It is caused by an increase in prolactin and can be a symptom of thyroid disease or, much less often,a pituitary tumor. See your doctor and get it checked out.

Yes, but: First, i would have a doctor check the discharge to see if it's milky (they can see that under the microscope). If so, i would recommend blood hormone testing (thyroid panel, kidney function, and prolactin level) and a breast ultrasound in addition to the mammogram. Also, there are many medications that can cause this, but not zyrtec (cetirizine). Are you on anything else? Or any herbal supplements?

Duct ectasia: Benign thickening within the milk ducts may cause some blockage of the ducts with a liquidy substance which can cause areolar discharge. This isn't necessarily related to nursing or pregnancy.  A gynecologist can help make this diagnosis. Avoiding areolar manipulation can help.  Other conditions can cause breast discharge as well and should be evaluated by a gynecologist.

Lactating breasts: could mean either pregnancy or hormonal imbalance.  I would make sure to check a pregnancy test, then if negative check it again 1 week later.  You do not want to have a pregnancy and not know it.  If everything is negative, then you have irregular bleeding from cycle and may need to see a GYN to regulate it again, especially after 3-4 months of similar bleeding.

Breast discharge: In early stages of pregnancy, some women notice clear discharge & in later stages it becomes milky. Also discharge can be seen following vigorous physical activity such as jogging. Other causes might be fibrocystic breast disease, galactorrhea (some causes are hormones or medications), infection, mammary duct ectasia in older women, breast tumor.  Your ob/gyn can explain more details.

Depends: It depends on the color of the discharge, if you are feeling a lump or if you have an abnormal mammogram. Bloody discharge needs to be evaluated by your doctor.  Be sure to get a yearly mammogram and clinical breast exam by your doctor.
