Sores in Nose: The white hard bumps could be milia which are superficial cysts under the skin. Sores in the nose are usually caused be a bacteria called staph. Prescription topical Mupirocin applied inside the nose is helpful for this skin condition.

Anatomy: Concha bullosae are enlarged middle turbinates, which is a variant type of anatomy,  causing nasal obstruction. Enlarged inferior turbinates also cause difficulty breathing. A deviated septum is a crooked divider between the right and left nostrils, also causing difficulty breathing. Swollen nodes might be a reaction to a recent or chronic infection. See an otolaryngologist for evaluation.

i have a "questionable cyst of the septum pellucidum"  it's 3.0cm(ap) x 1.3cm(transverse) x 3.4cm(right to left)  is that a big deal?

Depends: If it's symptomatic it may require surgery.  The symptoms would be related to increased pressure from the cyst blocking flow of the CSF (cerebrospinal fluid).  Google symptoms of increased intracranial pressure.  If you have symptoms like this, think about seeing a neurosurgeon.  It is normal in up to 15% of adults.

Probably not: That degree of septal deivation is serious if it causes nasal obstruction.  Depending on the status of other adjacent structures in the nose, 5 mm of septal deviation may cause little obstruction or it may cause significant obstruction.  If so, straightening it with a surgery called a septoplasty might be helpful.

ASD: Usually with an atrial septal defect the blood flow or shunt goes from left to right, however, in some situations when there is pulmonary hypertension the shunting goes from right to left.  The problem with that is it causes some of the blood to bypass the lungs and not get the oxygen needed to bring to the tissues.

Hypertrophic Heart: Hypertrophy means the walls of ventricle are enlarged and may be encroaching on the inside chamber of the ventricle.  This can inhibit outflow of blood to the body, which can be serious.  There is medication which can help this, and sometimes a cardiac procedure may be necessary.  It is very important to maintain good follow up with your doctor for appropriate treatment.

Could be: A septal deformity to the left could be your problem.  The fact that it does not switch sides certainly suggests that you have an obstruction.  However other possibilities are a nasal polyp, or other mass.  You need to see an ENT doctor for a diagnosis and treatment.

what procedures are available for 'patching' a perforated septum (quite large) left from surgery for a deviated septum?

Local tissue grafts: Typically they take fascial tissue eithe from your thigh or from a cadaver; elevate the lining on both sides of your septum and down onto the floor of your nose.  Then they put the fascial tissue over th hole on one side and tuck the edges under the elevated lining. They then slide the lining up from the floor of the nose and sew it in to place.
