Do nothing now: Baby is almost certainly perfectly OK. If renal pelvis 7.5mm, nothing to worry about &amp; will 99 - 100% settle down as a normal kidney. 7.5 cm is very large &amp; will almost certainly require intervention. Will require:- pediatric urologist, post natal US , VCUG &amp; nuclear scan after birth to evaluate swollen kidney for function. Removal only if no or essentially no function. Life with 1 kidney is OK.

She should be seeing: An oncologist can address your questions better.From what you have written, i suspect that your friend has cancer(?Renal-pelvis/ureter). We first need to know if this is a kidney cancer or a transistional cell cancer(tcc of the renal pelvis).
Surgery is the main treatment unless there is already spread of the tumor in which case some of the drugs like you mentioned(axitinib/afinitor) may be useful.

Need kidney test : you should have a nuclear medicine scan to see if your swollen kidney is blocked. If you have documented infections by urine cultures should be evaluated by urologist.
