Depends...: You need to determine what is causing your pain.  Podiatrists will examine and treat the cause of your foot problems but can also recommend proper shoes and if inserts are needed to avoid recurrence.  The inserts (aka orhtotics) may be over the counter or prescription.

Not heels!: Fibromyalgia, as defined by the american college of rheumatology, does not include foot pain.  That being said, patients with fms can still have foot pain.  I recommend a supportive shoe with a minimal heel, and arch support if you tend to "overpronate".  Pace your standing/walking with rest breaks. Find a skilled shoe-fitter that will take the time to find a shoe you are comfortable wearing.

Supportive: Any shoes that has support and fit confortably should be fine. Normal cadance and gait starts at age 3.

Soccer orthotics: Believe it or not - just like sneakers are specialized these days for every sport you can get orthotics the same way. 
Particularly if they are custom made - custom impying you can have anything you want. If you need help contact a podiatrist.

Spinal Fusion: I suspect your Back Surgeon will have some ideas for you.   Doesn't seem to me like Converse would provide enough cushion for support after surgery.

Rest isn't enough: The cause of the metatarsalgia has to be looked into. There may be something mechanical in how you are built and/or walk which needs to be addressed, possibly via the use of a custom orthotic. There can be other causes too, depending on the nature of the symptoms.  I'd suggest consulting with a friendly podiatrist.

what are the best shoe inserts for 5-inch heels that would last me for several hours?  i'm attending a party, & i normally wear flats.

Not always needed: I don't typically recommend a specific insert for heeled shoes unless a patient has specific symptoms.  If there are symptoms related to the shoes, sometimes a simple over the counter cushion insert can help.  More specific prescription orthotics would only be ordered as needed.
