The cremaster: muscle is an involuntary muscle whose main function is to help regulate the temperature of the testes by contraction/relaxation, and wouldn't normally be subject to "wear and tear" or injury unless there is direct trauma.  It is a thin muscle and under normal circumstances  difficult to image, but if focal pathology is suspected, ultrasound or MRI would be best, albeit limited.

: Not in the long term. One benefit to having duplicate organs like lungs/kidneys/testis is the other can provide enough reserve to make up for the loss of one.

Infection spreads: Epididimytis can spread to testicles if not treated. Usually it is treated and resolves. Both infections can be seen with ultrasound

See your doctor soon: Testicular pain (also called orchodynia or orchalgia) in a young man can be caused by several things.  Infection is common, but torsion (twisting) is, too, and it's potentially dangerous.   Tumors of the testicle do occur in younger men, so get this checked out asap.

Jockstrap may help: You should see your urologist. This is not dangerous. Support of the testicle with jockey shorts or a jockstrap may help.

Cremaster ?'s: Even with cremaster contractions, one or both testicles are not subject to body temperature (37 degrees C.) so sperm are not inactivated.  See a urologist for a sperm analysis to be certain.  Unlikely that infertility is caused by cremaster contractions.

: Perineal pain is not caused by chordee or cremasteric muscle. Pl see your MD or gen surgeon for cause. Good luck

Epididymitis: Without the benefit of a physical examination my thoughts of a possible diagnosis are epididymitis (an inflammation of the structure behind the testis), intermittent torsion (twisting of the testis), or an orchalgia (a pain in the testis).

Cremasteric muscle: No.  The cremasteric reflex naturally becomes diminished as a man ages.  The muscle that pulls the testis up is often also impaired following inguinal hernia surgery or scrotal surgery.
