A medium firm mattre: Most back pain sufferers feel better with a medium firm mattress, but some may prefer firm mattress.
Medium firm mattress will cradle your body while supporting it at the same time.
So it is neither soft nor hard.

Shoulder impingement: Causes of shoulder impingement are usually overuse, shoulder labrum or rotator cuff tears, and certain anatomic configurations of the shoulder joint. I wouldn't ascribe impingement syndrome to sleeping patterns or poor mattress.

Covid: Benadryl is an antihistamine that can actually be helpful during covid if you are having a lot of secretions, or congestion. In general it is fine to take it but if you are experiencing symptoms of shortness of breath or confusion then would not recommend it.

Up to 1 month: Vivitrol or long acting injectable naltrexone, which is used for alcohol dependence or opioid dependence, is a formulation that lasts 1 month.  Unfortunately, those side effects may last from 1 week to up to one month, the duration of the medication.

Sleepiness: Yes, excessive sleepiness can be a symptom of an underlying medical problem, such as sleep apnea, hypothyroidism, anemia, cardiac disease, diabetes.  You should see your primary care physician to have an evaluation and possible some blood tests to narrow down the diagnosis.

hard to say: Could be a simple cold or you could have a more serious issue.... You would benefit from seeing your doctor.
