Obvious symptoms: The aorta is the high-pressure, biggest artery coming out of the heart. A small fistula connected to it would have high pressure blood squirting out with each heartbeat. That means a person would be throwing up lots of blood or pooping out lots of blood.

The common types are between the trachea and esophagus, the rectum and the vagina or bladder, or the colon and the skin.  They are generally in the setting of either surgery and post operative care or infection, like diverticulitis.

Fissures usually cause a severe tearing like pain with bm and severe spasm in the sphincter after a bm and may have significant bleeding, fistula is usually a pressure type pain and may drain a mixture of blood and infected fluid or purulence. They are treated differently so a correct diagnosis requires an exam. Both may require surgery but not the first choice.

Perhaps: If one had significant pancreatic insufficiency and that they had chronic diarrhea, then perhaps one could develop rectal fistulas in the right scenario.  This would be rare and an exception.  Otherwise, the pancreas is far from the rectum and cannot be associated with this condition.  I would defer to my GI and colo-rectal and general surgery colleagues for their opinions.
