Unlikely HIV: These aren't common ways for ARS. The could be uncommon causes of end-stage HIV but even then it's not common to see just these symptoms. 
Muscle twitches and atrophy and according weakness of the affected side could be due to early age polio, impinged nerve in the spine or other nerve disease. It wouldn't normally occur bilaterally so I am guessing you mean just one side.

Compensation : Your symptoms are typical of compensation pain. Your right lower extremity is taking on most of the work, as your left side is unable to. 
Recommend therapy to strengthen the left side, and a clinical evaluation to determine why your right knee hurts. You may need an XRay or an MRI of your right knee.

Therapy: The best non-surgical option is therapy to work on muscle stengthening. You may want to be seen by a physiatrist (rahabilatation doctor) to determine any special modalities and/or medications that may be beneficial in recovery of the muscle atrophy.

Suggest: Sounds like you have significant injury to your knee, and the thigh atrophy is not unusual under the circumstances.  Would be an excellent idea to get some physical therapy while you await the MRI, and hopefully, you have already seen an orthopedic specialist.

: First. Your Piriformis muscle is located in the hip area not near the calf. It sounds has though you received a work up concerning this. Have you seen a neurologist ?  You give a condition of a muscle that has lost its nerve connection. How did this occur?  See a neurologist for care. A neurosurgeon might be able to reconnect muscle if not too late.

Depends: The muscle atrophy associated with temporary disuse typically is for that area that is immobilized.  It is doubtful that your proximal musculature - eg quads, gluts - will be totally limited.  The level of atrophy will be modest but will be dependent on a number of factors including genetics.  Anecdotal reports are that ingesting more protein/supplements may decrease the level of atrophy.

Needs evaluation: B12 is not toxic so high B12 due to taking it is not bad. If someone has high B12 without taking it then it can be a sign of serious  problems including liver &amp; kidney disease, myelproliferative disorders including leukemia &amp; polycythemia vera &amp; hypereosinophilic syndrome.
