None: Unless a hydro becomes infected there are no symptoms whatsoever. Hydro can be diagnosed on HSG, at surgery or "sometimes" by ultrasound. Good wishes.

i am a 62 year old menopausal woman diagnosed with a hydrosalpinx about 34x44 and 4.8. there are no symptoms. what should i do ?

Possibly observe: A small relatively simple adnexal mass can be observed. I will often do several ultrasound tests to make sure the cyst is stable and not growing, then check ultrasound once a year. What i want to make sure is that the cyst is not ovarian or tubal cancer. If a cyst doesn't grow, is small and simple, negative ca-125, then observation is an option i give my patients.

Maybe: That is a large hydrosalpinx. Often they are chronically infected and contact infected fluid inside. If it bursts open, it can lead to life threatening infection and hospitalization. However that is not very common. It may also cause you chronic pelvic pain or painful sex and will likely affect fertility.

Rarely, but may hurt: We mostly treat hydrosalpinx - a fallopian tube that is blocked at the end furthest from the uterus, swollen and fluid-filled - to improve fertility if 1-sided or both sides. Some women with a hydrosalpinx suffer from chronic pelvic pain, including painful intercourse, while others have no symptoms. A hydrosalpinx can get acutely re-infected causing pain and fever: pelvic inflammatory disease.

Infection: Infection is the more common cause of a hydrosalpinx. Other causes can be endometriosis and prior pelvic surgery.  It's "cured" in most cases by removing it to avoid fertility problems (getting/staying pregnant) and an increased risk of ectopic pregnancy.

DEPENDS ON HOW TREAT: Most are treated by removal to prevent pyosalpinx or infection.  Therefore not recur on that tube but can recur on opposite tube.

Low risk: Most hydros are from inflammatory damage, and are otherwise normal tubes. Cancer of the tube is very rare, but could theoretically present as a hydrosalpinx.

Accepted Answer

It depends: It depends on where in the Fallopian tube it is located, whether the far (distal) end of the tube is closed,  whether or not you are contemplating near future conception and what is the primary cause of the hydrosalpinx, most commonly infection.  All of these factors and more will determine "next steps" by the GYN MD.
