The best way to differentiate the two would be history of trauma, 
xray and orhopedic exam
obviously the appropriate treatment will be quite different based

: Sometimes a hairline fracture is missed on X-rays.  If pain persists, other tests (including MRI and bone scan) can determine if a fracture was missed.  Tapping on the fractured bone can help confirm a fracture; it would be painful.  It's best to see an orthopedic surgeon for this.

Intensity/duration: Broken bone often but not always associated with more intense pain intially.  Broken bone will aften preclude ability to weight bear through affected body part.  Displaced fractures (breaks) will make affected body part appear deformed (shortened, rotated or angulated finger for example).  Pain from broken bone may persist longer than sprain.

Can be, if fx was --: Relatively simple one, without needing any procedures, except splinting/casting for up to 6-8 weeks, and once healed + pt & that should result in a normal wrist.

Complicated fx needing appropriate treatment including surgery can also result in a normal or near-normal wrist by experienced hands.
