It is one cause: It can also be a sign that other issues are involved, like kidney disease or damage, liver disease, etc. It all depends on what is found in the urine.

NO: The darker urine is likely due to bilirubin from the liver also being bothered by the mono.  If you have not already seen a physician and had blood work done, you should do that.  Make sure to drink lots of water.  If the spleen ruptures, it is life threatening due to severe bleeding in the abdomen.

Hepatitis C: Decreased immune system and more bilirubin in urine causing dark urine does mean worsening hepatitis. You should see a gastroenterologist for testing. Memory loss could be linked to increase toxin build up from poor liver clearance due to hepatitis c.

No: A UTI will generally not cause any discoloration of the genital area.  You may have something else (like yeast) that can coexist with a uti.

Painful urination: It sounds like he has a urinary tract infection, though a sexually transmissible infection or STD is a possibility, since he is sexually active.

I will recommend a visit to your primary care provider for testing.
