when i have swollen tonsils there is this "gland" that sticks out from the bottom of my neck towards my tonsils. makes it difficult  to swallow. what is that? pitty i can't post a picture

Sore Throat: Viral sore throats usualy clear up on thier own within a few days. If your symptoms are severe or last longer see your doctor for poosible cultures and antibiotics. If this is frequent you should consult an ENT doc to discuss further options.

Reaction...: Your lymph nodes( glands) get swollen as a response of the infection. It means your body is fighting with what is causing your symptoms. If you are feeling better already it is most likely your symptoms should resolve in a few days. However, your glands may stay enlarged for a little bit while your body gets rid of what is seems to be a virus.

Dr. can check throat: New throat pain can be from a strep throat infection (antibiotics are needed), a cold or influenza (antibiotics usually not used), or from some other cause such as allergies or irritants. Usually, a person sees the doctor if the sore throat is bothersome, recurrent, or doesn't go away quickly (mainly to check for strep). A couple days of throat lozenges and Tylenol (acetaminophen) can lessen pain.

Culture: If the tonsils are red and swollen for 20 days and an antibiotic has been unsuccessful I would strongly recommend a culture of the throat to rule out a viral infection or a specific bacterial infection.  Finally, infectious mono needs to be ruled out.  No it should not get worse before it gets better.  You need a specific diagnosis. See an ENT doctor.

Viral illness.: Most cold sores are caused by the Herpes simplex virus.  Especially the first time you get the infection there will be other symptoms besides the sores like aching, sometimes headache or fever and or swollen glands.

Possible lymph nodes: Tonsils are lymph nodes--the structures that hold many cells of our immune system. They can get enlarged as a response to infection. There are glands in the neck often called 'swollen glands' when enlarged, so often the two go together and both can feel sore if inflamed. Strep throat and many viruses can cause this. Cancer is rare for what you describe, but i strongly recommend seeing your doctor.
