Breast : Breast cancer can appear as lumps or knots in the breast and with swollen lymph nodes.  However, these can also be caused by many other non-cancerous conditions.  You need to see a physician for a thorough examination to find out for sure.

Anatomical assymetry: This is hard because something sounds very wrong and i would want your son evaluated urgently.  Here would be my rule:  if he is having any difficulty breathing, is lethargic or has a fever then get him to the er.  If he is comfortable, able to communicate in full sentences and is his normal personality then you can wait to see the pediatrician tomorrow.

Musculoskeletal?: It's unlikely that your pain is due to a gallbladder issue since you had surgery over a year ago. A select number of patients have retained left over gallstones which may cause pain in which an ultrasound or bloodwork is needed to see this. Most often, your pain associated with movement is musculoskeletal and will improve with rest and time, and anti-inflammatory medications.

Is it black in color: Black moles are sometimes called Beauty spots. But they can be moles(if flat) and a changing mole needs a medical examination from a doctor. Since you have  noticed some swelling/irritation go see a doctor to make sure you do not have melanoma...a type of skin cancer that can occur even at a young age. Do not delay as it can be a serious problem, sometimes..most are benign and harmless.

Family practice : Is it painful or itchy. If painful have it examined. It itchy it may be treated with Benadryl (diphenhydramine)  and Hydrocortisone cream. If it looks like hives it's an allergic response to something. If if persists have your dr check it out.

See above for causes of gynecomastia . Rarely  ingestion of significant amounts of soy products can also cause gynecomastia. Would follow up with  MD

May reveal cause: Lymph nodes around your clavicle may indicate a lung problem, even lung cancer.  A chest x-ray would be the first step.  A chest ct scan would be a reasonable alternative but much more expensive.
