It : It may be scarring and fibrosis and as it was drfained once the surgeon may not want to do this again. Sometimes areas of swelling stay swollen ewvebn though the hematoma or seroma is gone.
Often splinting, therapy and compression with a glove or similar garment will help

the problem is that if once tries to remove the scarring it just reacumulates if it is done again too early. This is especuially true if it is due to trauma.

Cranial burr holes: Cranial burr hole evacuation of the hematoma. For large for acute hematomas, craniotomy may become necessary.

Blood: After mastectomy then reconstruction that gets a hematoma is caused by blood.  The blood vessels supply oxygen to the breast tissue.  This can occur with any surgery and will most likely resolve on its own.  It may require drainage.  Talk to your surgeon and they will discuss it with you.

Hematoma: This simply means a collection of blood. This is cause by bleeding into a space an the blood cannot get out. The body usually absorbs this blood over time.  How long it takes depends on the size

Hematoma: It's not extremely common but it does happen. Probably less than 5% of the time. It will go away in a few weeks to months.

Removal: To best answer your question, I would need to know how recent the Hip Replacement was and when the Hematoma developed.  This probably needs to be at least drained, if not surgically debrided
