: Infected tooth can cause referred pain to the ear and facial swelling on the same side but it should not cause swelling of the ear canal (external auditory canal). Take care.

: Try Ibuprofen for a few days. it could be tension headache. If not better in afew days pl see your MD for evaluation. Hope you feel better.

TMJ: This sounds like tmj, or temporomandibular joint, pain.  Often people experience associated bruxism, or muscle spasm of the chewing muscles, which radiates pain down the neck and sometime up into the side of the head.  Soft diet and anti-inflammatories, such as motrin, are helpful.  If this continues, seek the help of your dentist, or a TMJ specialist who will often recommend a bite guard at night.

Ear pain: Does it radiate?  Only one side?   Any facial symptoms?   If just behind left ear, could be ear infection, nerve pain as it is usually sharp, TMJ problem , or even trigrminal nerve problem.  Try rest, if progresses, seek eval.

Sprain: If your injury was severe enough to cause a head injury, it is likely you injured the ligaments between your head and your neck.  The mucles of the neck attatch to the bones of the head and can be very painful until healed.  A year is sufficient time to heal this pain with the physical therapy you have probobly undergone.  See a physiatrist for more help.  Consider prolotherapy injections.

Directly behind?: There are a set of lymph nodes that drain the scalp and the outer ear.... Or it could be an ear infection, or a scrape, or an ingrown hair... Have someone look at it to be sure.

Several options: If the sharp pain is triggered by touching or wind blowing on the area it might be a neuralgia of the occipital or upper cervical nerves.  If it just happens without an inciting event and is very brief then it may be difficult to elucidate a cause.  Sometimes it can be from muscle spasms in the area.  If the pain becomes more constant or ear symptoms develop then see ent.
