NSAIDs: Unless you're on treatment for peptic ulcer or gastritis or taking nsaids daily,  I don't understand why you should take a PPI whenever you take an nsaid. Just don't take them on an empty stomach since they do stimulate secretion of acid in the stomach.

PPI: PPI's reduce the acid in your stomach.  By reducing the acid in the stomach you reduce some of the digestive functions of protein breakdown and release of pancreatic enzymes.  Changing these digestive functions can affect the gallbladder.  However, this may not be the only thing contributing to the gall bladder dysfunction.

i have vitiligo and reduced thyroid function. recently i developed a peptic ulcer. will protein pump inhibitors have negative affects on my thyroid?

No: But chronic PPI use may have its own set of side effects. At the minimum, you will need to take magnesium supplement since PPI use decreases its absorption.

i am using voltaren (diclofenac) 75mg. and proton pump inhibitor for my stomach. how long should i wait to take voltaren (diclofenac) after i take the inhibitor?

No interaction.: The two apparently have no reported interaction, however, there are concerns about taking each of those two drugs for a prolonged period of time.  Of particular concern are the side effects of the NSAID drugs such as voltaren (diclofenac).  They not only can cause ulcers, but also salt retention, hypertension, heart failure and heart attacks.  Ppi's also can have significant side effects.

is a light cramp and stomach growling normal when taking proton pump inhibitors for acid reflux ?

No: This is either something else, or you didn't have reflux to begin with. I see a lot of patients who are put on ppis for upper abdominal pain, told they have reflux or an ulcer, and when the pain doesn't improve they finally get some tests and it turns out it was their gall bladder the whole time. Try eating pizza for dinner, then ice cream for dessert. If that brings on the pain, it's not reflux!

Yes, certainly: aspirin like drugs cause ulcers. The ulcers might heal without th euse of medications but they prevent the stomach from making acid and speed healing and prevent further troubles.

: In some patients, proton pump inhibitors (PPIs) may be taken just once a day to lower the risk of stomach side effects in patients taking NSAIDs. However, if s/he isn’t already included, it’s going to be good to loop your doctor into the decision to take a PPI. S/He may want you to take a different dose or avoid it entirely because it may affect other medications you’re taking.

No evidence of this.: Caffeine should not affect protein absorption much.  Since caffeine can cause things to move faster through the digestive tract, theoretically the time available to digest and absorb nutrients is decreased.  However, the body overall is very good as digesting and absorbing protein, so this change in speed should not affect much.
