Tetanus safe: If your last tetanus shot was less than 7 years ago, it is not necessary to get another booster now.  Your management with washing, etc., was appropriate but it is still possible to get an infection from bacteria on the skin whenever there is a cut or puncture.  Get an evaluation if the site looks red or becomes more painful.

stepped on rusty nail, there is very small puncture wound, and the skin is red. it didnt bleed at all nor does the wound even hurt. i had a tetanus shot 10 yrs ago, and a booster 6 yrs ago. is that enough?

: Maybe?  If the nail went in fairly deep, then you may want to stop in ER.  A tetanus booster within 6 years should be good for not getting tetanus. However, if the nail penetrated deeply enough they might give you an antibiotic just in case. Sometimes bacteria from your skin or the nail can cause an infection.  If it starts to get red or sore, see your doctor immediately.

Would expect it: You don't mention age but at any age that depth of penetration is enough to involve deep structures of foot ligaments/tissue. The chance of a deep tissue infection is real. Good cleansing of the wound &amp; close monitoring are important. A review of tetanus status &amp; booster if needed is important. If it went through a shoe or moist tennis shoe it may have really bad germs from that source.

Puncture wound: Puncture side may possibly got infected. Go to an urgent care or ER. You need tetanus injection if you are not up to date. May be an x-ray, antibiotic prescription and pain medication. You can not manage it by yourself. You can have serious complications.

Any wound a problem: Tetanus spores from the dirt can get into any break in your skin and potentially germinate. Over time the growing germs may elaborate the tetanus toxin and give you tetanus. The rusty nail scene is the one most talked about, but any dirty wound in a susceptable person can be a source.

Monitor for now: If you did not actually suffer a puncture wound, then no need to be treated.  However, if you did break skin and your immunization status is unknown, you may want to get it looked at for any signs of infection and/or to update your tetanus.

Clean/ tetanus: Cleanse the area well with warm water and soap. Rinse.  Rusty puncture wounds increase risk for tetanus. Obtain medical care and get a tetanus shot if you haven't had one in the last ten years.
