Poorer results: Chemo is used in an adjuvant and neoadjuvant situation, the latter to enhance treatment prior to resection.  In either case failure to respond to chemo means the tumor will become more aggressive and shorten survival.  At present other therapies including immunotherapy can make up for the chemo failure using newer drugs like the checkpoint inhibitors.

Tumor progress: Chemotherapy used in many different ways.  It can be used with RT to minimize recurrence after surgery and improve the procedure in  a neoadjuvant setting, used post op in an adjuvant setting to reduce recurrence and as a  therapeutic in metastatic Ca. While prolonging survival in mets, it has resulted in no cures.   Failure to  respond to chemo leads to progression and a terminal event.

Depends: The answer depends on the type of cancer you have.  Cancer cells are different in each body location.  There are different chemo medications as well.  If you do not respond, then another chemotherapy agent might be an option.  Surgery and/or radiation may be indicated as well.  Speak with your oncologist about options.  Don't be afraid to get a 2nd opinion by another oncologist.

my husband has stage 4 nsclc and has stopped responding to chemo. we have 3 children ages 17, 12 and 1. what should i tell the kids?

Prognosis: I am sorry to hear about your husband. Obviously if he has stage 4 ung cancer and not responding to chemo, we are talking about a very short life span here. Prepare your 2 elder kids by telling them that their father is soon going to go to heaven and they will reunite in life hereafter. Let them spend enough time with him. Get your priest involved or a grief counsellor and other family members.

Well....: Many lymphomas, and testicular cancer, are two forms which are famously benefited by chemotherapy. Speak to your oncologist, who can better discuss the benefits of chemotherapy for your individual cancer. Good luck.

? What is DCA?: I would love to help you but if you use abreviations(like dca!), i would not understand your question. So tell me the whole story and i will answer your q.
As for other treatments, stage-4 cancers are frequently incurable, so they tend to relapse and more treatment if often required. So be watchful and have a close follow up and ask the questions to your oncologist who can probably guide you as we.

The : The reason for the production of breast milk can be attributed to the hormone therapy. However the production of breast milk can also be attributed to stress and other medications.
Seeing as your body underwent the stress of surgery and is on hormone therapy, this side effect can be expected.

Treatment of mets: the ablation of these spots mainly helps improve the symptoms they cause .sometimes being small make it difficult to ablate..multiple spots also tell us there are other areas are affected but did not show up yet. finally ablation helps local control of the spot but usually has no effect on the overall survival.the will of fighting the cancer carries 50% of the cure.keep fighting

whats done for refractory chemo vomiting that doesn't respond to a cocktail of antiemetics? is the next step deep sedation to calm the vomiting center

Accepted Answer

Many drugs, can help: If you tell us the names of the antiemetic drug cocktail given so far, we can suggest some add on drugs. One drug that can help is marinol(not often used) and another one to use is Compazine (prochlorperazine) in high doses. Either of these would help.
Sedation is also important but they may have already use Ativan which is another good drug when regular antiemetics fail.

does well differentiated squamous cell nsclc usually respond well to chemo?  patient is healthy enough for surgery, if indicated.

Yes: When one defines a squamous cell ca of lung as protentially resectable and well differentiated then he or she is a candidate for possible cure by sugery alone.  When chemo such as taxol (paclitaxel) platinum is added the possibility of controlling any recurrence is very high.  Ratdiation occasionally has a role and now immunochemo is coming into the picture.
