Depends: If it is small or not causing nerve pressure or leading to weakness of the spinal column it can be observed. If it is causing such problems, it is removed by surgery. Depending on size and location, a fusion may be necessary. A spine surgeon can provide the options in an individual's case.

Usually not: Hemangiomas are benign blood vessel tumors.  Sometimes, if the tumors are quite large, blood platelets can get activated in these tumors and stick to the blood vessel walls.  This can result in a low platelet count.  Platelets are necessary for proper blood clotting.  This problem is called kasabach merritt phenomenon and is actually quite rare.  If it occurs, it could cause a bleening tendency.

Normal : As best I can understand your question, the MRI results are essentially normal.  A hemangioma seen in a MRI is usually considered an incidental finding that rarely has clinical significance.

Physical therapy: Appropriate physical therapy with back stretching, core strengthening should help. If not, a spine specialist is next, but avoid surgery.

how does one know if it's metastatic cancer to the spine or spinal hemangiomas? for yrs before i had breast cancer my dr said hemangiomas. now after breast cancer they are saying metastatic. i want 100% proof. two completely different things?

: Sounds very frustrating, as hard to objectify the comments.  Suggest this be handled via a second opinion.  Additional studies may well give you the "100% proof" you seek.

Common and not bad: Its the most common benign (i.e. good as opposed to cancer) tumor one can have in the bones of the spine, about 1 in 10 people will have one.  The can weaken the structure of the spine bones though can that can cause problems in itself but that should be visible on the MRI or X-ray that showed the hemangioma

Unusual indeed: Typically, a vertebral hemangioma is a congenital variation discovered coincidently, when image obtained for other reasons.  Would consider alternative explanations.

Get attention if new: In general if you have a new type of symptom that you've never had before such as the symptoms in the toes or the vibration sensation then it's important to go immediately to your doctor.  Hemangiomas can bleed.
