Goiter: Hterogenous thyroid can be due to simple goiter and may be associated with hyperthyriodism, hypothyroidism, thyroid nodule, thyroditis and thyroid cancer
if you were told not to worry by your endocrinologist means they have ruled out treatable conditions.

Thyroid ultrasound: Your thyroid ultrasound was ordered by your doctor to evaluate an abnormal thyroid gland.  Generally, we are looking for nodules or masses of concern.  Happily no masses were seen.  Heterogeneity is the characteristic of your gland showing variations in water content which would be expected findings in glands that are swollen or inflamed.

Hashimoto's disease?: Common cause of ultrasound pattern with hyperemia is Hashimoto's thyroiditis. Can initially be hyperthyroid and eventually becomes hypothyroid. Usually an autoimmune disease with thyroid antibodies, When hypothyroid usually have to have thyroid hormone suppliments.

: Ask the physician who ordered the thyroid scan about a radioactive iodine uptake test to determine whether any of the nodules are "cold" nodules, which may require biopsy.

No: Not cancer. It goes along with your thyroid disease. That is why you need synthroid (thyroxine)

Nodules: Usually multiple thyroid nodules are benign in contrast to a solitary nodule. I'm not familiar with such a description as noted in the right upper lobe. In this situation the attending physician should sit down with the radiologist and ask him/her what the possible significance of that finding is, what, if any, other testing may be helpful and what specialists, could be consulted for advice.

Thyroid: What have your doctors said about this. There are many options that include surgery or doing nothing. You can rely on your doctors for your management.

If your thyroid levels are OK and your thyroid gland is not very enlarged it would be OK to have nothing done aside from regular examination.

What tests?: You may have normal TSH and free t 4 but still have anti thyroid antibodies. That would be called euthyroid hashimotos. So "normal" thyroid test doesn't tell whole story.

May need follow up : The provider who ordered the study should have been able to explain the findings to you and whether or not you need any follow up. There is a possibillity you may  need a follow up test that looks at whether the areas of nodularity are active hormone producing tissue or whether they are cold spots in the thyroid. Cold spots often have to be looked at in more detail.
