Engorgement of tissu: Appears to be the engorgement- enlargement of small areas of breast tissue.As it is closer  to the surface near the nipple you are able to feel. If related to the menstrual cycle,indicates the swelling localized island of breast tissue in response  to hormonal factors of menstrual cycle. this is nothing to worry. Hot compresses /warm soaks may help to reduce pain, if very painful.

Apples & Oranges: What you feel on breast examination and see on mammogram are not always correlated; although mammograms are excellent screening tests, they do miss cancers. Therefore, the lump you feel could very well have been present last month and gone undetected. Please see your primary care dr or gyne--an ultrasound of this lump will help determine if biopsy is necessary.

Possible: At 36 you may hve infection (boil ) of periareolar   montgomery gland infection , must see your doctor to make sure have nothing serious, and if it is to drain (and antibiotics )   which will give  relief to your pain.

Lactating Lump: Even though you are not breastfeeding, your breasts did prepare to feed your new baby. Therefore, you still are having some milk production. The lump could be a clogged milk duct or an infection. Call your OB for him to evaluate.

for how long after benign lumpectomy scar tissue in the breast tissue itself can develop. after 1 yr 8x2mm hard/rubbery lump in breast. us was normal.

Scar tissue: and other post surgical changes following lumpectomy can evolve over a year or two, and sometimes longer.  If you have a new lump in the same region it could be scar tissue, but not necessarily.  It needs to be fully evaluated.  Besides ultrasound, mammography and possibly MRI could be needed.
