Yes: If there are sutures left, they can become irritating and infected. However, your lump may not be a scar, it could be a hernia. You'll need to see your surgeon.

No connection: Small bowel obstruction from scar tissue (or adhesions) is caused by scarring that is external to the small intestine, usually caused by previous abdominal surgery or infection. I am not aware that any special diet will increase or decrease the risk of this happening.

Lipoma not new 90Ds: It is unlikely that a new lipoma formed over a 90 day period. It is more likely to be due to a traumatic hematoma with resolving fibrosis or could even be a hernia. The only way to establish a definitive diagnosis will likely require diagnostic imaging and possible surgery.

Needs follow up: This is a complex situation with lots of possibilities. The fever should be addressed first since that could be caused by so many things. Then your gynecologist can assess the vaginal cuff. You may have granulation tissue, scar tissue, pelvic adhesions or a number of other issues which can be addressed.

We just visited : If your nipple got really purple, I will bet it is scar. Or it may well be gynecomsstia. Your cancer risk is near zero, and if this is cancer, it will enlarge or ulcerated. Show your physician on your next office visit or if you want it removed.
