Colonoscopy: If no pain, bleeding, or altered bowel habits (constipation, diarrhea), probably this is nothing serious. However, mucus production by the rectum or lower colon can indicate inflammation, infection, or tumor (adenoma). Also, you're at an age when routine colonoscopy is recommended to check for cancer and pre-cancerous growths like adenomas. See your doctor soon. Good luck!

Rectal discharge: Young woman with a white-ish rectal discharge when she strains and there is no B.M. This is mucus, and is normally produced by the colon and rectal lining, and can be increased when there is spasm, IBS and more rarely when there is inflammation or polyps. Without blood it is rarely a source of a serious problem, and a simple sigmoidoscopy or colonoscopy can rule those out.

Mucus thing: It may be related to something you ate or possibly so,etching structural like a polyp or material Ina diverticulum, a sac formed in the bowel. It may also have come from your vagina? I'm not sure how old you are but if this continues, be examined and consider a colonoscopy. All the best.

May be IBS or more?: Inflammation, infection  ibs may be associated w/ abdominal discomfort, altered /alternating stool pattern, accompanying nausea, bloating, gasiness. Often improved by stooling, ibs is defined by changes in perception of gut distension, motility, sensation. Diet ; emotional stress play significant roles here in patients with ibs, but first rule out inflammation, ischemia, ; infection. See doc pls.

Viral or diet: If a newborn, can be normal. If not can be a viral infection. Could also be dietary. If he is eating something that does not agree with him it can affect the bowels. He may also benefit form  probiotics.If it has been going on for over 10 days and  bothers you, then make an appointment with your pediatrician for more information.

NO: Stool may take on a slimy appearance for a few reasons. Most commonly, there may be some underlying colon infection especially if you have been around sick people, traveled abroad or taken recent antibiotics. It may also represent a problem with your ability to absorb fats. If fats are undigested they will produce a shiny slimy texture and the stool may float. Seek medical care if it doesn't impro.

Mucusy stool: Mucus discharge occurs in up to 50% of patients with irritable bowel syndrome, with or without pain. Mucus may also be produced in response to partial obstruction, chronic constipation, rectal dysfunction, infection, & even from secretory villous tumors of the rectum. At the very least, discuss your ongoing finding with your doctor who knows you best & can advise what treatment &/or testing may be needed.

Multiple possibility: Is the blood bright red? Have you had a history of stomach problems in the past? Do you have family members with stomach problems? Given the symptoms you describe, I encourage you to get a physical exam, as the blood, pain and reflux at your age could mean a chronic abdominal disease that needs to monitored, or something more worrisome.

No other disease?: Each person with constipation needs to be sure there is no other disease causing it. If constipation is just due to slow movement of the food and poop through the gut, then the things to try are vegetables, water, prune juice, prunes, fiber-with-water, polyethylene glycol, Senokot or other senna, milk of magnesia, other laxatives... A doctor can evaluate and maybe prescribe helpful medicines too.
