Many symptoms...: While this could be a sign of a herniated disc, it sounds a little more like facet arthritis as it does not radiate to different areas and does not have a constant dull ache. That being said, a physical exam and imaging would be necessary to be sure. If you are concerned about an actual herniation, then I would recommend seeing a physician for an exam to be safe.

Yes if bilateral: It is possible. If symptoms are unilateral on face then a more careful neurological examination and probably imaging e.G MRI of brain may be needed.

Cause?: You appear to be asking for the cause of your symptoms.  As you have identified, it may be your anxiety/panic attacks.  However, depending upon symptoms it might be a disk in your cervical spine.  It may also be a mild concussion even though you do not feel the blow was hard.  You need to be seen to make this differential diagnosis.  Conslut your doctor.

Probably not: Pain or numbness in the left arm or hand can be associated with heart attacks. However, there usually are other symptoms such as chest pain, shortness of breath, and fatigue. If you just have numbness and tingling it is probably related to pressure on the nerve either in the neck, the elbow, or the wrist areas.

Yes: It is a tendonitis and it mostly causes pain of the thumb with apposition and extension. If the inflammation is bad enough it can compress the nerve that arises at the dorsum of the wrist and can cause numbness and tingling in fingers innervated by that nerve.
