Sjogrens: A rheumatologist is who you need to see. Usually, medicines to suppress your immune system are used. These can include low dose chemo meds, immune modulating drugs or prednisone.

Rx of Sjogren's: Most rheumatologists  use hydroxycholorquine based on dosing of 6mg/kg of body weight.  I also use auranofin with most patients. I treat fatigue and mental fog with modafinil.  Some uncommon but serious problems that can occur in sjogren's have recently been treated with IV rituxan, (rituximab) with reasonably good results.

Yes: There may be as many as a million folks with this, most of them undiagnosed. A person who must sip water first-thing-in-the-morning has dry mouth for some reason, and this and obvious dry eyes need a workup (labs in this case) though there's no magic pill for sjogren's as there is for many other illnesses.

Sjogrens: Make sure you get an accurate diagnosis - it is very specific based on lab work or a lip biopsy.  Then treated symptomatically based on severity of symptoms.  Artificial tear drops for dry eyes.  Drink plenty of water for dry mouth.  There is 'artificial saliva' and medication available called secretagogues (mild cholinergics) to stimulate natural salivation.

Yes: Upper GI series will show dysphagia but it wont necessarily show the cause.  In sjogrens disease the cause of dysphagia can be due to dry throat, but can also be caused by inflammation in the brain stem.  The latter might also cause other symptoms besides just dysphagia.

Dry eyes can be from: different conditions that are manageable, and can be easily treated locally. Whereas, Sjogren's is part of an autoimmune disease spectrum involving severe dryness of the eyes, and can involve joint disease, and dry mucous membranes as well and is system wide. Management is more complex.

Big difference: Sjogren's syndrome is caused by an autoimmune disorder and decreases the aqueous or water portion of your tear film. The tear film on the ocular surface has three layers: oil, water (thickest layer), mucus (front to back). Dry eyes are more common and are usually caused by excess evaporation of the tear film (usually due to decreased oil layer and decreased adhesion of mucus layer as we age).

Check it out: It may be that this is an enlarged lymph node which is common in lupus/Sjogren syndrome due to the hyperactive immune system. Still you need to check this out with your local physician in case you need further work up.

Autoimmune disease: Sjogrens is an autoimmune disease. This means that the body starts to " attack" itself due to an alteration in the ability to identify its own tissue. It " perceives" the target tissue as a foreign invader and tries to eliminate it. This leads to signs and symptoms of whatever organ is being attacked.  Like many autoimmune diseases, there are genetic factors that predispose someone to be affected.
