Possibly tendonitis : Does hurt when stress your biceps?  If so might be biceps tendonitis where biceps tendon connects to anterior shoulder.  Press on front of shoulder & see if you can arouse pain.  There are about six or seven muscles & one could have tendonitis.  See an osteoparh for specific diagnosis and therapeutic advisory.  Sometime a single cortisone injection, if skillfully placed, can resolve the problem.

Need exam: I assume u are throwing a baseball . After an elbow injury ( mcl , fx?) that took a year to recover your pitching mechanics may be @ fault.See a good pitching coach/ instructor and have him watch you throw... Core muscle strength is not an uncommon cause of secondary shoulder pain. Of course, rotator cuff issues @ age 37( also biceps anchor/ labrum='slap') may be present. See 'sports' ors !

TOS: 30's person has "pain that extends from shoulder to elbow to hand". This symptom constellation suggests neuropathic disorder; compatible with Thoracic Outlet Syndrome.  Consult an Osteopath. In my experience, too many Neurologists doubt reality of T.O.S.  A down sloping shoulder with weak ipsilateral superior trapezius muscle can be an offending bio mechanism causing brachial plexus impingement.

Your : Your neck, shoulder and left arm pain may be from a cervical disk herniation or stenosis.  I would recommend seeing your primary care doctor.  You will probably need an MRI scan of your cervical spine.    Patients may benefit from anti-inflammatories, physical therapy, steroid injections or surgery depending on the specific cause. Rotator cuff: Your symptoms of pain @ night and pain w/ movements away from body suggest your rotator cuff is involved. Pain can be from the rot cuff getting 'pinched' underneath the point of the shoulder( impingement syndrome).Try taking a NSAID for a few days. If your pain continues see a board certified ors for examination/ x-rays . Best of luck.!

Unlikely: Usually ganglion pain is localized to the affected wrist. However if it is affecting movement and one is compensating for the pain it could cause referred pain to other joints on the same extremity.
