Rarely: Rarely testicular atrophy  (shrinkage ) is possible after complex inguinal hernia repair, if blood supply is cut off  by constricting the cord that carries blood supply, especially older patients. It is a known complication of hernia repair.

: Localized inflammation after surgery can be a common occurance. So can localized temporary changes to surrounding tissues. Discuss these changes with your surgeon at a follow up appointment as they know exactly what the surgery involved and can follow the healing process.

Sometimes: swelling of the scrotum or testicle occurs after hernia surgery. More swelling is common if it was a larger hernia or if the hernia was very scarred internally. Sometimes purple discoloration is also seen. This is not unusual &amp; is temporary. Contact your surgeon with any concerns &amp; follow up with them to be sure you are healing properly.

Post Hernia repair: When you have hernia repair there is trauma to the tissues and surrounding tissue. This trauma leads to swelling and even possibly some edema and pain.  Post operative swelling and pain should subside quickly. This sounds normal so far.

Swelling, etc.: Inguinal hernias are holes in the abdominal wall at the same place where the blood vessels and nerves to the testicle travel.  Trauma to these structures in the course of hernia repair can lead to temporary swelling and tenderness following surgery.

This is common: Hernia surgery involves putting mesh in the location where the spermatic cord goes to the testicle. The swelling and inflammation that occurs after this surgery can irritate this cord causing you to sense pain in the testicle. This should resolve by 3 months after your surgery.

Yes: Usually the pain gets better with time. If it does not then it may indicate nerve damage. If the pain is persistent then he needs to see a pain md. Local injections may help.
