i've been to a dermatologist who  said it looks like i have a basal cell carcinoma on my face, just above my lip. should i get a second opinion to confirm what it is? can i go straight to a plastic surgeon for a 2nd opinion and have it removed?

Basal cell CA: Depends on your insurance and network.  If the dermatologist is of the opinion that this is a basal cell it probably is. Excision is the way to go so seeing the surgeon is the best thing to do.

Painfulll scar: The plastic surgeon will evaluate the scar for hypertrophy, keloid or the possibility of neuroma. If it is a neuroma than revision surgery may be indicated. If it is just a hypertrophic scar or a keloid then treatment may include intralesional steroid injection or combination of steroid and 5-fu, massage or light therapy. Please follow up with a boar certified plastic surgeon for proper evaluation.

Depending on your skin and exact circumstances your facial plastic surgeon or dermatologist may recommend laser treatments (including co2, 1550 nm erbium), peels, dermabrasion, or well as topical treatments.   Seek a physician with experience.
