Please don't : Please don't stop breast feeding. It is the absolute best for your baby. Talk to your gynecologist about pain to see if you have a clogged lactating duct or even an infection. In most cases the pain can be relieved with warm pads applied to your breast and with gentle massage.

Probably OK: The glandular tissue in the breast, as well as the connective tissue between the glands, does get stretched a lot during breastfeeding. Most likely you are feeling these normal breast changes. But if your breast becomes red and swollen, or pain becomes significant, it may mean an infection. In that case, you should see your OB doctor.

after a week on antibiotics i still have a sharp/shooting pain in my right breast. is this more than an infection? it gets worse after breastfeeding.

BREAST PAIN: If your breast is infected, you should not breast feed. Infection will go to the baby. Please check with your md.

Trauma: Sounds as if you pulled a muscle or tendon beneath the breast.  If the tenderness does not recede significantly in the next week or two, go and get examined by a breast surgeon or a gynecologist.

Age: As part of question age is important.   At 42 female heart not high on list. Multiple possibilities.  Cervical nerve entrapment. Peripheral nerve entrapment.  Intrinsic shoulder problem with referred pain down arm.   Mass deep in "arm pit".  Muscle spasm.

Chest pain: Considering  your medical issues, it could be several entitities, but the more concerning ones relate to your lungs and heart. I would consider going down to the emergency room as soon as possible, because only a complete history, physical with testing is going to  be appropriate for your complaint.
