Referred pain?: The nervous system sometimes registers referred pain because of connections made very early in development. The back of the throat causes ear pain, for a common example. A problem almost anywhere in the front or back of the lower abdomen can register as pain in the lips of the vagina. For example, a kidney stone, or a problem with an ovary.  If continues see your practitioner.

Is it muscle?: Would definitely need more information for this one. When and how did it start, how long has it been like this, was there an injury, any nausea, fever, diarrhea, blood in urine? How does the muscle spasm in back explain the abdominal pain? Right or left side? You should see a doctor - if severe, go to the emergency room. If muscle spasm, use heat, ibuprofen, and gentle exercise.

Bladder spasms?: Probably should still get a urine test to be sure no infection. If crampy, sharp and intense pain, particularly if bladder very full, or just emptied (especially when was very full) may be bladder spasm. Try to urinate more often and at slightly slower pace for couple of days and see if pain resolves.
