Urachal cyst/sinus?: Pain in the belly button/navel which may radiate down to the bladder or pubic area could be from an abnormal urachus that has a cyst or sinus present and is irritated. Urinary complaints and drainage from the umbilicus (belly button) are other associated symptoms. See a general surgeon, for further evaluation. Labs and imaging may be normal even with a small sinus or cyst present.

Among other  possibilities, you may be having an incarcerated umbilical hernia or a tear in the tissue around the belly button that opens and closes depending on the strain that you impose on it, so it is painful when stretched. Get it examined if it feels serious

Some thoughts...: Abdominal pain has many causes.  Far too many to list here.  When you see your doctor, they'll ask you lots of questions: is it worse before or after you eat? Are your bowel movements normal? How long has it been going on?  Any nausea? Does eating make it feel better?  The "differential" for abdominal pain is so big that you really deserve a thorough history and exam to get to the bottom of it.

Urinary issues such as cystitis and pylelonephritis are other contributors. If debilitating issue certain physician should be consulted for thorough exam and possibly testing utilizing some imaging and blood tests.
