Context: Is the d dimer normal or high for the lab? Is there a history of smoking cigarettes, or a history of blood clots or emphysema? Chest pain, coughing up blood?  This sounds like lung problems, but could also indicate a blood problem. More clinical info is needed.

See your doctor: Anemia from your rectal bleeding is high on my list of diagnosis. You have to see your doctor asap. The low red blood cell count (anemia) may be causing all your symptoms. Go in today if you can. The reason for your rectal bleeding needs to be investigated.

Black stool: Your red cell indices are consistent with iron deficiency or blood loss. Black stools, if they're not caused by taking iron or bismuth containing  products , are signs of bleeding from high up in the gastrointestinal tract. Usually the causes are bleeding peptic ulcer or acute gastritis. You must be evaluated by a gastroenterologist and undergo a gastroscopy for proper diagnosis and get treated

See a Doc: Anytime a patient has shortness of breath they should see their primary care physician. There are many causes of shortness of breath and you should see your primary care physician to make sure there is nothing serious causing those symptoms. Your blood pressure was relatively normal at the time of those symptoms.
