Yes: Nerve irritation from the lumbar spine can refer pain/numbness/tingling to the scrotum. The most common causes of nerve irritation are disc problems, narrowing of nerve canals (stenosis) and cysts.

No: Pre-ejaculate has nothing to do with sciatica.  Sciatic nerve pain is usually starts in your spine.  However, the nerves traveling from the penis to your spine usually come from levels s2-s4 which start in your tailbone area.  If there is actual back issues going on, that might be the reason for your sciatica.  If this does not resolve, speak with your doctor to evaluate the spine/sacrum areas.

i hurt my left sciatic nerve by injuring my back. i have had numbness in the bottom of my foot, back of my thigh but , today in my left testical.  it's numb and it's uncomfortable. my left leg is numb as well. should i be concerned??  thank you

Get this evaluated: You describe involvement of the S-1 nerve root, and now it appears your symptoms are progressing.  If you have a damaged disc, there may be increasing compression of nerve roots, and if this is case, you have risk of increased weakness, involvement of bladder, and perhaps trouble with urinary function.  This is not a situation to delay.  Contact a surgeon, and decide on therapeutic intervention.

Urologist can check: Panadeine appears to be a foreign pain medication, similar to the tylenol+codeine in the U.S.  Since it is a narcotic, the goal would be to wean off of it as soon as the pain is better. The urologist can best predict if the pain will return, and how long irritation or inflammation of the nerve might last. A neurologist can also give a helpful evaluation, if it is indeed a nerve problem.

: The main thing to worry about with ongoing testicular pain, besides the possibility of a tumor, is testicular torsion.  This is when the testicle twists enough, that it cuts off, or limits, it's own blood supply.  If the pain is still present, I would have it evaluated very soon.  Frequently, ultrasound is used to identify blood flow to the testicle. It may also spot other possible abnormalities.

Testicular pain: A urologist may be able to detect a cause that has been missed. Perhaps a pain med with an anti-inflammatory component (e.g. Vicoprofen) would provide better pain relief until the urologist has thoroughly examined your case.

USUALLY disc disease: The sciatic nerve can 'get pinched' by a bulging disc in the low back.  The mechanism of injury for bulging disc is usually some type of lifting injury with 'bent over posture' or twisting.  I had a patient 'sneeze so hard' he developed a completely weak leg and had surgery the same day.  Usually in men, another cause of sciatic compression is sitting on a wallet causing pain 'down the leg!'.
