Worse breathing?: Pattern: worse lying down and better sitting up (especially leaning forward)-very suggestive of pericarditis which would not show up on a ct angio. Pain worse with deep breath? (usually would be). Sle is a common cause. Echo is indicated to r/o effusion. Sed rate is commonly up. Colcrys (as you list) is good - taking 0.6 mg bid? Nsaid or steroid pulse may be necessary. Pain won't respond to butran.

mild biapical pleura and parenchymal scarring. calcified granuloma seen in each lung. 
cause? should i be concerned? treatment? off and on chest pain into.back.  muscle/joint pain. not sob. heart tests all ok?

Need to examine: It is not feasible to provide a meaningful opinion without additional history, physical examination and may be some tests.  What you described is consistent with pulmonary tuberculosis, not necessarily active disease. It would be prudent to see a doctor, preferably a chest specialist/pulmonologist.

i have lots of chest pain and family history of heart attacks at young age. i've had ekg nuclear stress and echo.should i still worry its continuing?

Dont worry: You should not worry  but you should get it further evaluated to determine the cause.   Common conditions would include costochondritis, chest wall muscle spasms,  referred pain from gall bladder, and artery spasms.  You should get a holter or other continous ekg device to make sure you perform an  ekg during an episode of the pain.

im young and have mild chest pain. went in for an ekg and it came out as lvh with repolarization abnormality and qrs (t) contour abnormality? does this mean heart failure?

: It doesn't mean heart failure but it indicates presence of cardiac structural pathology. Do you have a history of heart murmurs or hypertension?  Family history of cardiac abnormalities? An echocardiogram will shed more light on your cardiac abnormalities. Consult a cardiologist for evaluation. Appropriate testing will be ordered.

i have been suffering with chest pain. increase in heart rate. pins and needles down my left arm. did have svt when i was younger. has it returned?

Possibly: Or it may be panic disorder or hyperventilation. A physician can examine you, obtain an ekg and determine the diagnosis.

: Your EKG detected nonspecific ST elevation and T wave changes, which are often misinterpreted as "infarct" by the machine.  This EKG strip has to be read by cardiologist in the context of clinical scenario.

Not sure of your que: Not sure what you are asking. There are many causes of chest pain, aside from the heart. Chest pain from lung disease / pneumonia, acid reflux in the food tube (esophagus), rib fractures, &amp; muscle strain of the pectoralis muscle are but a few examples. Pain is sensed by the nerves (vagus nerve especially) surrounding the heart, but those nerves go to different organs, too, which can be confusing.

Many things: There are several causes for chest pain near the heart. The most serious include unstable angina/impending heart attack, heart attack (myocardial infarction), aortic dissection, and pulmonary embolism.
 Other causes include pericarditis, esophageal spasm, esophagitis, costochondritis, or gerd.
 
Since many of these can feel similarly, and timing is vital, prompt medical evaluation is important.
