Simple: The spermatic cord has to be freed from hernial sac , that will result in scar formation.

Not really  : I'm sorry this is happening. Groin pain a known possible consequence of this surgery.  Most surgeons will not operate on just pain -too much risk (and can make it worse) .  A neuroma is often the cause, or a nerve impinged by a suture inside. Most imaging will not help  define anything.  Hope it gets better.

What kind of hernia?: It depends on the location of the hernia and what kind of surgery was done. Surgery for a hiatal hernia or diaphragmatic hernia may cause scar near the heart. But even this should not lead to a mass between the heart and lung.

PAIN after HERNIA: Surgery persists ~6-8% of patients most often related to scar tissue irritating nerves. It's  common in younger pts ; in those with significant pain before surgery. Certain activities including ejaculation can cause/increase the discomfort. Treatment is usually conservative with pain management ; physical therapy. Surgery is limited to those with severe pain. Start with an exam by your surgeon.

Determining the cause of post herniorrhaphy pain should begin with a comprehensive evaluation and examination by a surgeon or hernia specialist.

Possible.: The open repair nowadays include using mesh, very often is ine the form of a cone, the vas deferns is in that area , it lifted withe rst of the spermatic cord with a penrose tape and it lies over the mesh, this could cause scaring that would include the vas which carries the ejaculate and i might get narrowed causing the pain.

Probably scar tissue: The testicle obtains its blood supply and drains the sperm thru the spermatic cord, that runs through the inguinal canal, which is mobilized as part of the hernia repair.  The pain during ejaculation is likely due to some scar tissue around the spermatic cord as part of the hernia repair and is not likely due to hernia recurrence.  If very symptomatic, reoperation may be necessary.

See answer : If have not done, would recommend see a urologist and get their opinion on cause of your ejaculation pain as scar tissue would be an uncommon cause of ejaculation-only related pain post-hernia repair. Most nerve entrapments accompanied by pain(s) unassociated with ejaculation. If you're correct, would reserve repeat surgery as a last resort. Most such pains improve with time. Nerve block an option.

had surgery to remove scar tissue from a femoral hernia repair done 12m ago he went in the same cut some movements feels like ripping on the inside.

Could be more scar: I would anticipate a slow recovery.  Take it easy.  Massage the area.
